Personal Case Management
Kanabec County Community Health case management can help when you are faced with a difficult decision for a family member/friend. We will assist you with understanding the different options, resources and services available to you.
Services include:
Individual Evaluation and Care Planning:
Our team will work with the individual and caregivers to development of short and long term needs, based on physical, social, emotional and financial factors.
Plan Implementation:
Once a care plan has been developed our staff will assist with arranging and coordinating services.
Home & Community-based Services Waivers:
Home and Community-based services help to meet the needs of people with disabilities and older adults.
