All Minnesota community health boards are required to take part in a community health assessment and planning process. This helps local public health agencies identify the most important health needs in their communities and focus resources where they can have the greatest impact.

Kanabec County Community Health completes a Community Health Assessment (CHA) every five years as required by the Minnesota Department of Health. The CHA provides a snapshot of the health of Kanabec County. The process includes reviewing health data, community conditions, available resources, and other factors that can affect the health and well-being of residents.
To complete the CHA, Kanabec County Community Health works with community partners and residents to collect, review, and understand local data. This information helps identify health priorities and guides decisions about how to improve health across the county.
The findings from the CHA are used to develop a Community Health Improvement Plan (CHIP). The CHIP identifies shared health priorities and helps Kanabec County Community Health, community organizations, and residents focus their efforts on areas where they can make the greatest impact.
The CHIP is a collaborative, living document that grows and changes as community needs, resources, and efforts evolve. It brings together the programs, partnerships, and initiatives happening across Kanabec County that support the identified health priorities.
By documenting this work over time, the CHIP provides a shared record of how organizations and community members are working together, where resources are being focused, and the progress being made toward improving the health and well-being of Kanabec County.
Kanabec County Community Health is currently working on the 2027-2032 Community Health Needs Assessment and Community Health Improvement Plan.
