This program is a fun and easy way to learn about your child’s health and development. This service provides ideas about what to teach and at what age to teach it. Questionnaires are mailed out every 4-6 months until your child’s 3rd birthday. This program includes personalized information about fun activities for you and your child to do together too.

• Every few months, you will get a form asking how your child is playing, talking, growing, moving and acting.

• Fill out the form and send it back to us.

• You will get the results with fun activities for you and your child to do together.
